New Jersey Student Symptoms Line Listing Mobile App

Given the nature of a learning environment and the potential exposure rates, a school or childcare facility is highly susceptible to an infectious disease outbreak.
How does it work?
Carefully tracking illness at the school will lower the risk of exposure. The New Jersey Student Symptoms Line Listing app is a tool for a school to report any incidence of communicable diseases to the local health department. With the app, a health investigator at the school can record the number of students an illness and then describe each individual case by checking the signs and symptoms and conditions of each child. Using the form, health investigators can list the student's name and then check off all present signs and symptoms of the infection including fever, headache, rash, or fatigue. The health investigator will also document whether the child has seen a physician, was admitted to the hospital, and any diagnosis or lab results.
