The HIV AIDS Adult Case Report app is used to document cases of HIV/AIDS infection in patients over the age of 13 as part of a communicable disease surveillance program.
How does it work?
A healthcare provider will populate the form with patient identifier information and history, patient demographics including diagnosis status and date diagnosed, and the facility of diagnosis. With the app, the healthcare provider can indicate and describe any risk factors and organize all laboratory test results in HIV antibody tests, positive HIV detection tests, and indicate infection diagnosis along with patient notification status. The app includes a checklist that the provider will use to check any AIDS indicator disease and then note the current treatment program. This case report from the app is a vital tool for collecting information from people living with HIV or AIDS for control of infectious diseases.
