There are five screens for the application, including the following: employment status, other health insurance, Medicare, authorization and signature. You can collect basic patient information, patient employment information, employer name, phone number, retirement date, spouse employment information and spouse employer name, all in a timely manner from your desktop or mobile device.
This app also provides a signature page to certify that the information provided is true and correct.
