The form will provide information about the owner, designer, the beginning and end date of construction, and the General Arrangement Plan or a Pocket Plan. The owner will also provide all information regarding the construction site, hull number and class designation, hull material, freight class, cargo hold, type of propulsion, dimensions of the vessel, and other information needed depending on the type of vessel. The type of inland waterway vessel can include passenger vessels, tankers, barges, tug boats, ferry, cargo vessels, patrol boats, and other type of transport or work boat.

The owner will provide all certificates and documents for compliance for tankers, dry cargo ships, pushers or other vessels, pollution prevention plans, safety certificates, operation certificates, tonnage certificates, and documents providing that all domestic rules and regulations are followed. The owner must also give the full designs of the vessel, hull, cargo hold, and any other special equipment needed for the inland waterway vessel. All technical data for the vessel should also be submitted.
