The Kentucky Request for Conviction Records app will allow for an applicant to release all criminal records to an agency.

The applicant will provide their full information including their full name, social security number, gender, race, and date of birth. The name of the agency or organization for the criminal history and conviction records to be released to must be provided.
The applicant must allow the Kentucky State Police to provide the agency or organization named with the full records of any conviction found in the Kentucky centralized criminal history record information system. A fee for all records must be provided to the Kentucky State Treasurer.
