Functional Assessment Interview Form Mobile App

The Functional Behavioral Assessment mobile app, or FBA, is used to better understand the pattern and purpose, or function, of a behavior. The goal of an FBA process is to identify events that precede (antecedents) and follow (consequences) a behavior. Careful functional analysis of antecedents, behavior, and consequences provides insight into the purpose or function of a behavior.

Use of this app on an iPhone, iPad, or Android device will help you administer the FBA process to a student who has been exhibiting inappropriate behaviors. You can easily document any disruptive behavior using direct observation and other behavior assessments. each functional analysis is completed using open-ended questions that will help build a rapport between the student and analyst. All completed behavior intervention plans are saved in the GoCanvas Cloud and can easily be shared with others as a PDF.
