Parent Consent for Student Random Drug Testing Mobile App
As a parent or guardian of an enrolled student, they use the app to read and understand the school districts policy regarding voluntary random student drug and alcohol testing.
How does it work?
A parent or guardian can use the signature capture app feature to sign and give my consent for their child’s enrollment in the extracurricular activities or on-campus driving privileges. Signing this consent, they are also enrolling their child in a random drug testing program. The Parent Consent for Student Random Drug Testing mobile app allows you to replace your paper consent form with an Android, iPhone, tablet or Windows desktop computer. The mobile form includes fields to document items such as student name, student ID, grade, campus and captures student and parent/guardian signatures. Once the mobile app is filled out a secure PDF is generated. The PDF is easy to share, and a copy is stored in your account for your substance abuse assistance records. You can also use the App Builder to personalize and customize this mobile form.
