Student Random Drug Testing Consent Form Mobile App
Core features are free forever
Schools can have a parent or guardian of a student enrolled in their school read and acknowledge the school policy regarding random student drug testing.
How does it work?
If a child participates in extracurricular activities and/or receives a parking permit allowing him/her to park his/her vehicle on school property during the school day. The parents or guardian understand that their child will be asked to provide a urine sample for drug analysis. By signing the app they consent to such testing as part of the District’s drug and alcohol testing policy. The Student Random Drug Testing Consent Form mobile app allows you to replace your paper consent form with an Android, iPhone, tablet or Windows desktop computer. The mobile form includes fields to document items such as student name, student ID, grade, campus and captures student and parent/guardian signatures. Once the mobile app is filled out a secure PDF is generated. The PDF is easy to share, and a copy is stored in your account for your substance abuse assistance records. You can also use the App Builder to personalize and customize this mobile form.
