The Office Furniture Request Form is a form designed to be used by universities to request furniture for specific departments, and also to remove existing furniture where necessary.

There are sections to list the requisitioner and department, location and contact details. A long text section is included for a description of the furniture requested as well as furniture to be removed (if necessary), followed by the requisitioners signature and the signature of the approving chair, director, or vice president, and a date when the any assessment will take place.

The second section of the form details various office items and each can be checked with a quantity that is being requested - these are broken down into private office (pedestal desks, storage cabinets, bookcases etc), lobby/lounge (loveseats, occasional tables etc), classroom, receptions, and miscellaneous. The form is fully customizable to suit any needs.
