How does it work?
Capital equipment has a minimum cost of $5,000 and a useful life of one year or more. Use this asset disposal form for the purposes of proper, documented property or asset disposal based on procedures, involving disposal of equipment and materials, whether it is surplus equipment or waste disposal.
Please note that anytime you dispose of tagged equipment, you should complete the form to adhere to proper disposal procedures and maintain accurate equipment inventory. This form may not grant approval to dispose of equipment; it only serves to update the inventory database. You should obtain, and keep on file, written approval from your deans, chairpersons or department administrators to remove capital equipment, however you may need the approval of the Asset Management Accountant.
