New Hampshire Lyme Disease Case Report

The New Hampshire Lyme Disease Case Report Form app is for case reporting to support infectious disease control and prevention. With the app, a healthcare provider will document health facility details and patient information. The app provides an organized checklist of signs and symptoms reported in the current episode of the illness including erythema migrans, joint swelling, and any neurologic issues. The physician will also indicate if the patient is pregnant, has been hospitalized, and has reported a prior diagnosis of Lyme disease. Because Lyme disease is infectious, it is important to capture the exposure history including whether a tick bite has been reported within 30 days of onset of the infection, whether the patient traveled within that same time frame, and relevant laboratory results.
