The Measles Exposures Tracking Form is used to document contact with any individuals who have had contact with people with measles and who may also be at risk of measles. With the app, a health investigator will record the name and basic information of contact with potential exposure to measles, the date of contact, immunity status, symptoms watch dates and onset of any symptoms of measles. The app user can make additional notes or recommend actions based on the status to minimize measles cases.
