Carefully read each statement and decide if your answer is “No” or “Yes”. When the words “drug abuse” are used, they mean the use of prescribed or over-the-counter in excess of the directions and any nonmedical use of drugs. The various classes of drugs may include cannabis (e.g., marijuana, hash), solvents, tranquilizers (e.g., Valium), barbiturates, cocaine, stimulants (e.g., speed), hallucinogens (e.g., LSD) or narcotics (e.g., heroin). Remember that the questions do not include alcohol or tobacco. The Drug Use Questionnaire (DAST-10) mobile app allows you to replace your paper questionnaire with an Android, iPhone, tablet or Windows desktop computer. The mobile form includes fields to document items such as date, clients name, DAST questions, DAST-10 Score and severity of the drug problem. Once the mobile app is filled out a secure PDF is generated. The PDF is easy to share, and a copy is stored in your account for your substance abuse assistance records. You can also use the App Builder to personalize and customize this mobile form.
