The Parental Leave Request (UK) app features a simple to use template in which parents can fill out a request to employers. The app asks for: personal details (employee name, job title, staff number, etc.), dates of parental leave, declaration, and signatures. Declaration options are as follows: I am named on the child’s birth/adoption certificate, or I have/expect to have legal parental responsibility for the child, the child is below the age at which the right to parental leave ceases, and/or the time off work will be to care for the child or make arrangements for the child’s welfare. The app can be saved for recordkeeping.
