The CACFP Home Monitoring Form app allows child care monitors to record information and details of meal times and interactions between the child and adult during a home visit.
How does it work?
This app was designed to meet CACFP monitor log requirements in Tennessee, but it can be modified for use in any location.
The CACFP program form automatically populates the review date, review times and can obtain GPS data. Users can add their own comments and notes regarding food choices, child eating habits and overall food nutrition. This information helps ensure that all children who are a part of state-funded nutrition programs are receiving the snacks and meals they need to stay healthy, and that they are learning about healthy habits as they grow. Once the CACFP monitor log has been completed, the child care and food program monitor can add their own notes and sign off on the log entry electronically.
