A backflow prevention assembly is a device that is used to protect a potable water supply from contamination from chemicals or biological hazards that may end up in a facility/building's internal water system if somehow the system is subject to reduced pressure. In water supply systems, water is normally maintained at a significant pressure to enable water to flow from the tap, shower, or other fixture. Water pressure may fail or be reduced when a water main bursts, pipes freeze, or there is unexpectedly high demand on the water system (for example, when several fire hydrants are opened or a sprinkler system is activated). Reduced pressure in the pipe may allow contaminated water from the soil, from storage, or from other sources to be drawn up into the system. This form covers a range of information including the facility information, nature of the work being done concerning backflow preventer, and instructions surrounding the information needed in the application.

A backflow prevention program ensures that proper testing of a building's backflow assembly, the repair/installation of one if necessary, and other important items.
