Notice for Multiple Problems (Canada)
The Notice of Multiple Problems form is to be used by contractors and/or subcontractors that are indicating problems arising from working with a general contractor or other stakeholder in a project.

Specifically, this form is indicated for use when multiple problems have arisen during the execution of the contract, such as changes in terms, owners barring access, defects in products or materials procured by owners, changes to construction plans, deficiencies in work of other parties, or extra work injected into the construction project by the owners.
Accordingly, this notice indicates that there have been problems which have arisen, and those problems have the potential to drive costs up, create delays, negatively impact all parties, and potentially cause construction defects.
