It acknowledges that the client has had the opportunity to ask questions of the piercer, and goes through checkpoints including whether the client is pregnant or nursing, does not suffer from skin conditions, has made the piercer aware of any allergies, that the client is not under the influence of drugs or alcohol, that the piercing is the choice of the client alone, and that infection is always a possibility.
