Participation in scouting activities involves the risk of personal injury, including death, due to the physical, mental, and emotional challenges in the activities offered. So ensuring the participants, guardian and/or parents sign approval that they are aware of the dangers of any activity is critical.

Include on the form that information about the boy scout troop activities may be obtained from the venue, activity coordinators, or the local council. Let them know that participation in these activities is entirely voluntary and requires participants to follow instructions from each adult. They must also abide by all applicable rules and the standards of conduct.

Use the Boy Scout Informed Consent, Release Agreement and Authorization to allow each parent or guardian to complete a consent and a detailed medical form.

Signatures can be digitally accepted to the consent form.
