In this online form, the app prompts you to spell out the conditions for say, a video release of performances or voice recordings. It covers the rights to performance, voice, likeness, and image. It also includes fields for digital signatures for both the talent and the representative, so your document has legal standing.

If you wanted to publish pictures you'd taken, of a school performance, in a local paper, it would be wise to have at least an actor's photo release form, and possibly a release of liability form, signed by each participant. If a child is under 18 you will also need the signature of their legal guardian. The release waiver wording should make it clear that you have full approval to publicly display a image or picture and the person will hold you harmless for sharing in whatever manner is described specifically in the form.
