With the New Jersey Patient Symptoms Line Listing (Respiratory Tract Infection) app, health care providers can keep accurate records of patients diagnosed with a respiratory infection and submit to the health department. With the form, a health care provider can compile a list of patients with a respiratory infection and the symptoms of the illness that each patient exhibits. In addition to the onset date and temperature of each patient, the health care provider would use the checklist to identify and record for symptoms of the disease. Signs and symptoms of the infection include nasal congestion, cough, sore throat, and fatigue. The health care provider can indicate if the patient received a vaccination for influenza. The app includes a section for notes such as hospital details and other clinical information. With this app, the health department will be equipped to monitor the diagnosis and treatment of patients with respiratory tract infections.
