A physician or other health care provider can use the New Jersey For Reporting Reportable Communicable Diseases app to report cases of infectious diseases to the local health department. With the report form app, the health investigator will describe basic patient information, health and immunization status, the onset date of illness, and associated symptoms. The health interviewer will document laboratory results with any positive cultures or antigen tests that confirm infection and the type of specimen collected. Other serology tests should be listed, dated, and include results. This report form app is a useful part of a health department's infectious disease surveillance program to monitor and mitigate disease outbreak.

Don't take our word for it...

“I’m not a tech guy and I was able to pick this thing up in probably an hour, and actually start to build apps. I’ve done them over lunch when I’ve gotten frustrated with a process that didn’t exist in our company. So for non tech people who’ve got a problem, the support here is awesome and I’d recommend it to anybody, not just in our industry.”
