With the Oregon Confidential Morbidity Report app, a health clinician can easily record and provide all required information about a reportable disease to the local health department. With the app, the clinician will provide basic data about the patient and more detailed information about the disease or condition including the onset of the symptoms, the diagnosis and any lab results supporting or confirming the disease.
