Oregon Confidential Morbidity Report Mobile App
Oregon law requires health clinicians to report confirmed or suspected diagnoses of specified infectious diseases and conditions to the patient's local health department.
How does it work?
With the Oregon Confidential Morbidity Report app, a health clinician can easily record and provide all required information about a reportable disease to the local health department. With the app, the clinician will provide basic data about the patient and more detailed information about the disease or condition including the onset of the symptoms, the diagnosis and any lab results supporting or confirming the disease. With the form, the reporting health providers can indicate whether the patient has been notified with lab results and if the local health department should speak with the clinician before interviewing the patient about the disease or medical records. Contact information for the reporting health clinician should also be included. With this app, the health department can promote patient's care and protect public health from potentially infectious disease.
