Kansas Measles Report Form

The Kansas Measles Report Form is a questionnaire used to gather data about potential cases in order to prevent the spread of the disease. With this app, a health interviewer can conduct household interviews to collect information on potential cases of measles. The health interviewer will begin by recording demographics and clinical information about the health of the individual such as the date of onset of symptoms, if complications required hospitalization, and laboratory testing for the measles virus, including if the disease was confirmed. By conducting a health investigation, the app user will gather details about the signs and symptoms of the illness including rash, fever, cough, and if the individual was vaccinated. The health interviewer will gauge a sense of exposure and risk factors about the potential source of the infection, who the person with the suspected case of measles may have come in contact with, and if others are at risk of exposure because of travel.
