Caused by uncontrolled electrical disturbance in the brain, a seizure happens suddenly, but should be accurately documented to promote the health of a patient. The Seizure Observation Log app is used for general communication among stakeholders who support the well-being of a patient who may experience a seizure. The app user should complete all sections for any seizure that may occur. With the form, the app user can record the date and time of the seizure onset, the length or seizure type, and all seizure observations. The form includes a list of possible observations that led to the detection of a seizure including sudden stare, sudden onset nausea, labored breathing, unconsciousness, convulsive activity, or vision problems. This app is a critical communication tool for someone in care of a patient with seizures to share what they observed with other caretakers.

There are a variety of seizure types including tonic-clonic seizures or grand mal seizures and petit mal seizures, also known as absence seizures, and various other types. Observing the seizure onset and seizure activity and/or brain activity can help identify seizure cues, and provide insight into treatment to prevent seizures such as medication, the use of a ketogenic diet, gene therapy, or surgical procedures.
