Once the mobile app is filled out a secure PDF is generated. The PDF is easy to share, and a copy is stored in your account for your animal control records. You can also use the App Builder to personalize and customize this mobile form. Please complete and return this to the Humane Enforcement Department at the address listed in the app. Failure to state the specific date, time and duration you observed the dog/s barking excessively will result in insufficient information for our officers to respond. Any others who are having a problem with barking dogs at this location will need to fill out a separate form of their own. Please read and understand the barking complaint information letter prior to filling out and submitting this form.
