The mobile form includes fields to document items such as date, business name and location, management commitment, training and supervision, and reporting. Once the mobile app is filled out a secure PDF is generated. The PDF is easy to share, and a copy is stored in your account for your OHS Safety records. You can also use the App Builder to personalize and customize this mobile form. This assessment was developed by the New South Wales Government WorkCover and amended for New Zealand use. Select the items on the app that most applies to your workplace. The sections with the most Items selected will identify your rating and show you whether, or how much, you need to make improvements. The app includes questions such as the following: Manager and worker responsibilities clearly identified, understood and acted on. Workers are always involved in decisions and developing policy and processes. Policy and processes followed in day-to-day operations. All workers are supervised to ensure policy and processes are followed. Survey results indicate that the majority of bullying incidents are reported. All reports are acted on. Return to work plans are implemented for injured workers when required and are effective in getting injured workers back to work.
