While caring for the patient, communication between the dialysis facility and long-term care unit is essential. The Renal Program and Long-term Care Communication Record app is meant to foster effective communication among a patient's care team members. The communication record is divided into two parts to be completed by the long-term care unit and the dialysis unit. The care unit can report any changes since the last dialysis treatment including a change in vascular access, vital signs, and lab work with new findings. The dialysis unit can record all dialysis information, blood pressure and other vital signs, and physician orders and concerns. Both parts include a section to keep a record of documentation included. Using this communication record for documenting a patient's health data for both the dialysis clinic and long-term care unit should be an essential part of the patient's treatment plan.
