The Laboratory Equipment Decontamination Confirmation Form app should be used by laboratory personnel in the decommissioning process who are aware of the hazardous materials used as they clean and decontaminate equipment such as fume hoods, refrigerators, freezers, etc., before contractors or facilities team move or dispose of the equipment. Laboratory personnel will need to share information about the equipment owner, identify the types of hazardous chemical, biological or radioactive materials used on the equipment. Laboratory personnel should outline the decontamination process, including the detergent solution used to decontaminate the equipment, hazardous waste that it has been in contact with, and if a disinfectant has been used. The equipment owner will need to sign the app before the equipment can be moved or decommissioned.
