Facilities must report the total numbers of units and/or aliquots of specified blood products transfused each month.

When reporting aliquots, the units from which they are made should NOT be counted as a transfused unit. The total number of patient samples collected must also be reported on this form. Monthly Reporting Denominators should be entered within 30 days of the end of each month.
