This app for a work permit should be completed by a student looking for a job outside of school hours. The app will require the student to provide detailed information about themselves, including age, gender, height, weight, distinguishing features, social security number, and a signature from a parent or legal guardian. The employer will also need to complete part of the app, outline the job responsibilities and agree to limit the number of hours the student will be working, will confirm the student will not be required to work hours during the school day, and that the student will not be compelled to work in any hazardous occupations. A physician will need to sign the app, confirming the student is healthy enough to perform the tasks required of the job. Additionally, an officer from the school the student attends will also need to sign the app. The student should expect to provide proof of age, such as a birth certificate.
