This includes the Institutional Care Program (ICP) and Home and Community-Based Services (HCBS) waiver programs. The Florida Department of Elder Affairs Informed Consent AHCA MedServ 2040 app allows users to give consent to: agree to an assessment to identify the need for long-term care, and to determine if user needs can be met in the community instead of a nursing facility, and authorize DOEA staff to access medical records. Users must understand and agree that the DOEA may need to talk to doctors and other health professionals. Users must also understand that they may need to interview family members, close friends and social services professionals about the situation. The app makes it easy to give consent and start long-term care services.
