The Recycle Feedstock Certification offers a straightforward checklist but it can be adapted to mimic your current paper version and documents items such as applicant/grantee name, grant number, quantity in tons, feedstock type and primary feedstock supplier signature. This Feedstock Certification form is a required application, progress report, and payment request document. The applicant/grantee is responsible to ensure the form is accurate and complete by each primary feedstock supplier (i.e., generator, processor, contractor, or hauler) who will provide feedstock for the grant project.
