The Authorization for Release of Driver Record Information app is a form that drivers can submit to the Department of Motor Vehicles to gain access to their driving record. The app has applicants understand: their employer may enroll the driver in the Employer Pull Notice Program to receive a driver record report at least once every twelve months, or when any subsequent conviction, failure to appear, accident, driver's license suspension, revocation, or any other action is taken against the driver's driving privilege during employment. The app also notifies applicants that enrollment in the EPN program is in effort to promote driver safety, and that driver license reports will be released to employers to determine driver eligibility as a licensed driver for employment.
