is one of the first forms you must fill out when a recordable work-related injury or illness occurs. Together with the Log of Work-Related Injuries and Illnesses (OSHA Form 300) and the accompanying Summary (OSHA Form 300A) these forms help the employer and OSHA develop a picture of the extent and severity of work-related incidents.

Within 7 calendar days after you receive information about a case, decide if the case is recordable under the OSHA recordkeeping requirements. Determine whether the incident is a new case or a recurrence of an existing one. Establish whether the case was work related. If the case is recordable, decide which form you will fill out as the injury and illness incident report.

Note that your employees have the right to review your injury and illness records. For more information, see 29 Code of Federal Regulations Part 1904.35, Cases listed on the log are not necessarily eligible for workers’ compensation or other insurance benefits. Listing a case on the does not mean that the employer or worker was at fault or that an OSHA standard was violated.
