The use of this app generates a report of contracts awarded for a project. All awarded contracts over $5,000 must be reported to the Department of Fair Employment and Housing Office of Compliance. One report per each contract must be submitted to the Compliance Office within ten days of the award date. The information of the contractor awarded a contract and providing services is collected in this app, as is the information of the project stakeholder.
The full contractor information including name, address, contact information, company officer, and FEIN should all be listed. The details about the contract including the type of contract, location, project number, contract amount, and estimated dates of the project will also be included. The awarding agency information should also be provided including the agency name, address, and title of the person completing the form.
