The full details of the contractor and construction project should be provided including the contact information, project number, contract number, and the dates of the project. The recycling process types or disposal activities used on the site should be noted, along with the type of materials generated.
The type of materials, type of recycling activity, quantity of materials, and the facility the materials will be sent to on the form should be listed on the form. The contractor will also provide the quantity of disposed materials, the type of materials, type of activity used, and the facility or landfill used for the disposal. The total materials reused, recycled, disposed of, or generated and the landfill diversion rate calculated will all be provided on the form.
