The full details about the project including name, project number, location, service agreement number, and contractor should all be provided. The description of the proposed service change and the reason for the change should all be detailed. The total cost of the service agreement and whether there is sufficient funding within the project budget should be provided.
If the new service agreement requires more funding than has been provided, the new fund appropriation team and amount funded should be completed. The construction administrator will prepare the proposed service agreement for recommendation for approval. If approved by the official, then the official service agreement amendment can be prepared.
