The individual must provide the name of the individual's agency, company representative, or other agency or company name for the records to be released to. Signing of the consent form provides the agency or company the authorization to run background checks and to receive all criminal history the state of Georgia has.
The individual providing consent must also provide their full details including their full name, address, date of birth, sex, and social security number. This authorization provides agencies the ability to provide background checks on the individual or employee investigations. If fingerprint cards are also provided with the consent form they will be processed through both Georgia and federal agencies.
