Details of the agency, business, or individual requesting the criminal history should be provided. A fee for all records must be provided to the Oklahoma State Bureau of Investigation.
The subject being searched must have their full details including their full name, address, sex, date of birth, and social security number provided. If applicable any requests requiring other documents for the Oklahoma Child Care Facilities Licensing or the Oklahoma Department of Corrections Sex Offenders Registration list must be complied with.
