A copy of this form is provided to the tank system owner/operator. The owner/operator must submit a copy of this form to the local agency regulating UST systems within 30 days of test date. The form covers an inventory of systems tested/certified, including in-tank gauging probes, piping sumps, electronic line leak detector, fill sump sensor(s) etc. These can be checked for a number of different tanks. Dispenser checklist items include dispenser containment sensor(s), shear valves, and dispenser containment float(s) and chain(s). There is also a testing/servicing checklist, in-tank gauging / SIR equipment, and line leak detector checklists.
