The form has entry for the name and address of the insurer, broker, insured, and additional insured. Details can be entered for various policies - Commercial General Liability, Builders' Risk "All Risk", and Installation Floater "All Risk", and for each the policy number, inception date and expiry date, limits of liability, and deductible. There is an additional section to detail warranties, endorsements, and exclusions. Finally the form is signed by the insurer's officer or authorized employee, along with date and telephone number.
