The 10 points cover areas such as readability of the operating procedures, safety warnings, and load capacity, the proper operation of lift controls, if there is any deformation or structural wear of lift components, contact points, or wires etc, if there is evidence of hydraulic leaks, unusual noises or movements, or any cracks around anchor bolts.

The final page of the form is a daily sign off where the week of and specific day are checked off and signed by the operator.
