This is a simple form that facilitates florist orders from customers, particularly over the phone or a wire service, such as FTD. Floral orders for delivery or customer pick-up are both easily handled by using this app. This florist sales order form automatically calculates sales tax and gives a total order value. In addition to delivery details, item descriptions and pricing, the app captures payment method, such as credit card or cash. For those special occasions, there is even a place to add a message for a card. You could supplement this application with a data base of your florist products, prices etc.

The 1st screen is the Delivery Information screen which allows a florist to enter the delivery date, day of week, AM or PM delivery, delivery name or location, hospital room and delivery address.

The 2nd screen,the Wire Order Information screen allows data entry related to the method of receiving a floral order or of sending one. Fields include whether the order is sent or received, the name of the wire service, the Florist's name, code, contact phone and complete address.

The 3rd screen is the Description screen. It has check-boxes to indicate that the order is one of the following: Arrangement, Plant, Cut, Corsage or a Piece.

The 4th screen, the Order Items screen, permits entry of the product description, quantity and price. An amount is automatically calculated. This screen permits entry of multiple products by clicking the Done button to add another item.

The 5th screen is the Order Summary screen, which shows the order amount, permit entry of delivery charge and phone or service charge and sales tax rate. The sales tax and total amount are automatically calculated

The 6th screen, the Card screen, is a handy feature that lets florist type in a message for the card that will accompany the order.

The 7th acreen is the Payment Information screen. It is here that such information, such as cash, C.O.D., new account, credit approval, shop charge and credit card type can be indicated.

The 9th and final screen is for Credit Card Information. If a credit card is used for payment, this screen captures all of the necessary information, including order writer, credit card number, expiration date, cardholder's name, phone number and address.
