If you don’t have these figures, you can use the information in the app to estimate the numbers you will need to enter on the Summary page at the end of the year.

You may use OSHA Form 300A Summary of Work-Related Injuries and Illnesses mobile app or an equivalent form. Some state workers compensation, insurance, or other reports may be acceptable substitutes, as long as they provide the same information as the OSHA 301.
