For infectious disease control and prevention, schools may adopt immunization requirements as recommended by the US Center for Disease Control or local public health departments. The Measles Mumps and Rubella (MMR) Documentation Form app is an ideal template for colleges and universities to collect immunization records from incoming students. Students must submit proof or a signature from a doctor or clinic for one of four options: two doses of MMR or Rubeola vaccine and the date the vaccine was administered, proof of the having had Rubeola Measles by a healthcare provider, or a rubeola blood titer test as evidence of measles immunity. The final option is signing a measles immunization waiver acknowledging the risk of exposure to the infection if declining the MMR vaccine. The document can be submitted to the student health services.

“I’m not a tech guy and I was able to pick this thing up in probably an hour, and actually start to build apps. I’ve done them over lunch when I’ve gotten frustrated with a process that didn’t exist in our company. So for non tech people who’ve got a problem, the support here is awesome and I’d recommend it to anybody, not just in our industry.”
