The animal is suspected of having rabies. Any animal suspected of being rabid shall be isolated in strict confinement under the proper care and observation of a licensed veterinarian in a veterinary hospital, animal shelter or other facility approved by the Police Department. Such animal shall not be killed or released for at least ten (10) days, for dogs and cats, fourteen (14) days for all other animals, after the onset of symptoms, unless permission is obtained from the Police Department to euthanize the animal for the purpose of laboratory examination. The Animal Quarantine Notification mobile app allows you to replace your paper notification with a handheld mobile device like a smartphone or tablet. The mobile form includes fields to document items such as case number, date, animal description, sex of the animal and captures the signature. Once the mobile app is filled out a secure PDF is generated. The PDF is easy to share, and a copy is stored in your account for your animal control records. You can also use the App Builder to personalize and customize this mobile form.
