The mobile form includes fields to document items such as State, school year, state agency name and address, contact information and special circumstances. Once the mobile app is filled out a secure PDF is generated. The PDF is easy to share, and a copy is stored in your account for your food manufacturing records. You can also use the App Builder to personalize and customize this mobile form. This annual interagency report collects data elements from the State agencies that administer the Supplemental Nutrition Assistance Program (SNAP) and from the State agencies that administer the National School Lunch Program (NSLP). This app collects data elements necessary to compute direct certification rates for comparison with certain benchmarks required by 7 CFR 245.12(b). The benchmark for the school year is 90%, and the benchmark for every school year thereafter is 95%. The app is intended to promote transparency and to strengthen the direct certification process so that States can monitor their own performance using the same measures and methodology that FNS will use, this report identifies each of the data elements and its role in the formula to calculate a State’s NSLP Direct Certification Rate with SNAP, even if, like Data Element #1, it is not collected in the app.
