The Formal Hazard Assessment and Control app is a risk assessment form that allows the user to put together a list of tasks, associated hazard identification of risks with those tasks, and a three point scale that evaluates the severity of the risk, the likelihood of the risk occurring, and uses that to perform a hazard analysis, by calculating the severity of the risk coupled with the likelihood that it occurs.
