The Health Plan Choice Survey aims to gather information about the health plan choices parents have made for their children and the parents' satisfaction with them. Apart from a ZIP code, there is no other personally identifiable information which is collected (e.g. name, address, name of company, etc.). The survey also collects information about the source of insurance coverage and reasons for choosing a particular plan. Although the main target of this survey is parents' who are still in the process of considering various health plan options for their children, this survey can also serve as an invaluable gauge of existing choices that parents have made.

Currently 1/5 Stars.	This application proves especially useful for field work, owing to the simple information input method. Almost all of the information that is entered is in the form of checkboxes which bypasses the often clunky keyboard interface of many smartphones, making entering information both easy and quick. Nonetheless, at least the optional ability to add additional information which may not fit neatly into one of the checkboxes (e.g. an insurance company which is not listed under the boxes) would be appreciated to be able record additional information.
